The most common triggers include:

  • Repeated pressure on anal veins from straining during bowel movements
  • Low hydration or diets lacking in fibre
  • Prolonged sitting, such as desk jobs or long commutes
  • Pregnancy or obesity, which increase abdominal pressure
  • Genetic predisposition or naturally weak venous walls
  • Many of my patients also report flare-ups associated with stress, irregular routines, or a diet high in refined foods.

Types of Piles

  • Internal Piles: Located inside the rectum; may bleed but usually painless.
  • External Piles: Form near the anal opening and can cause swelling, itching, or tenderness.
  • Prolapsed Piles: Internal piles that protrude outside the anus during straining.
  • Thrombosed Piles: When a clot develops inside a pile, resulting in severe pain and swelling.

Symptoms of Piles

  • Bright-red blood on stool or toilet paper
  • Itching, burning, or irritation around the anus
  • A small lump or bulge near the anal opening
  • Mucus discharge or sensation of incomplete evacuation
  • Pressure or discomfort during sitting or bowel movements
  • Possible Complications if Untreated
  • Gradual blood loss leading to fatigue or anaemia
  • Worsening prolapse and persistent swelling
  • Secondary infections or thrombosis
  • Chronic irritation that may lead to anal fissures or fistula formation

Diagnosis

Diagnosis is usually straightforward and done through visual examination, Per- rectum and Proctoscopy. In my practice, I use high-resolution proctoscopy to precisely grade piles and rule out any coexisting anal conditions. Occasionally, colonoscopy may be recommended for patients with bleeding or recurrent symptoms.

  • Diet rich in fibre from fruits, vegetables, and whole grains
  • Adequate water intake to ease bowel movements
  • Avoiding straining and maintaining regular bowel habits
  • Warm sitz baths to soothe discomfort

2. Modern & Surgical Treatments

Laser Haemorrhoidoplasty:

A minimally invasive, day-care procedure where laser energy precisely shrinks haemorrhoidal tissue. Patients experience minimal bleeding, rapid healing, and can return to routine activities the next day.

Mucopexy (Stapler / Ligasure / Conventional):

Mucopexy is ideal for prolapsed or large piles. It lifts and repositions the anal mucosa to its natural location. In my practice, I often combine mucopexy with laser treatment for precision, less pain, and long-lasting results.

Open / Conventional Haemorrhoidectomy:

Reserved for advanced or recurrent cases not suitable for minimally invasive procedures. Though effective, it requires a longer recovery period.
