Soft Tissue Swellings

Lipoma and Cyst Care

Soft tissue lumps are common findings in daily surgical practice. Most of them are non-cancerous and grow slowly beneath the skin. Even though they are usually harmless, it’s important to have them examined by a specialist to confirm the diagnosis and prevent future complications.

1. Lipoma – The Fatty Lump

  • A lipoma is a soft, rubbery swelling that moves easily beneath the skin.
  • It results from the overgrowth of fat cells.
  • Usually painless and slow-growing.
  • Seen most often on arms, shoulders, thighs, back, and neck.

Management

If the lipoma stays small and symptom-free, observation is enough. When it becomes larger, tender, or cosmetically noticeable, it can be removed safely under local anaesthesia through a small incision.

2. Sebaceous or Epidermoid Cyst

  • A sebaceous cyst is a small, dome-shaped lump containing sebum or keratin material.
  • Common sites include scalp, face, neck, and back.
  • It can get infected and discharge thick, foul-smelling material.

Management

The cyst is best treated by complete surgical excision along with its capsule. This ensures that it does not recur. If it’s inflamed, antibiotics or drainage may be needed first.

3. Pilar (Trichilemmal) Cyst

  • These cysts develop from the hair root sheath, mainly on the scalp.
  • They are firm, smooth, and sometimes appear in multiple numbers.
  • Though painless, they can interfere with combing or styling hair.

Management

Simple surgical removal provides permanent cure and prevents recurrence.

4. Ganglion Cyst

  • A ganglion is a fluid-filled swelling that forms close to a joint or tendon, most commonly on the wrist or ankle.
  • It arises due to leakage of joint fluid.
  • The lump may change in size and sometimes disappear for a while.
  • Repetitive wrist activity can make it more prominent.

Management

Rest and compression can reduce its size. Persistent or painful ganglions can be treated by aspiration or surgical excision.

5. Dermoid Cyst

  • A dermoid cyst is a developmental swelling that contains trapped skin tissue.
  • Usually found near the eyebrow, neck, or tailbone area.
  • May contain hair, sebaceous glands, and skin debris.
  • Present since birth and grows slowly with time.

Management

Surgical removal is the only permanent solution and helps avoid infection or enlargement.
